You need to bring a Sulfuron Ingot to the Bar in BRD and turn it in to the Thorium Brotherhood NPC for the recipe to craft the epic portion of the hammer. The mats are listed above. The legendary portion is made by combining the eye (drops from Ragnaros) with the epic portion. It takes some time farming the Molten Core to complete. Any Blacksmith can craft the hammer.
